We are so glad you have made the choice to enroll your child in the Parkview School District! For your convenience, we now offer online enrollment. Please complete the following steps to begin your student's enrollment process. 

Enrollment Forms and the Enrollment Process

Please complete the enrollment paperwork and contact Jenny Steinke, District Registrar, to complete the enrollment process. Enrollment forms must be completed for all new students entering the Parkview School District.

Step 1:  Complete the Enrollment Inquiry Form 

Step 2:  Complete Immunization Record Form

Step 3:  Students enrolling in 4K or Kindergarten are strongly encouraged to complete the following forms:

Physical Form

Dental Examination Form

Vision Examination Form | Spanish Vision Examination Form

Step 4:  Contact Jenny Steinke via the email link to set up an appointment to provide the child's birth certificate, a parent/guardian picture ID and proof of residency to enroll a student.
